CHICAGO – NURSE PRACTITIONER invited to join growing Skilled Nursing team caring for adolescents through seniors

Chicago, IL: We seek an experienced NURSE PRACTITIONER to join a growing private practice with offices just west of downtown Chicago. This progressive medical practice employs a multi-disciplinary team of more than two dozen Physicians and APPs committed to patient-centered care, providing outpatient primary care and specialty services as well as inpatient care in acute care hospitals and skilled nursing facilities.

The Skilled Nursing team based in Oak Park is ready for another Nurse Practitioner. Confident NPs certified as FNP, Adult NP, or AGNP are all equally welcome. Providers in this group deliver care to patients in acute care hospitals, skilled nursing facilities and outpatient clinic settings on the north side and near west side Chicago suburbs. Patients range in age from adolescents to seniors and geriatrics. This is an excellent opportunity to enhance the quality, safety and efficiency of services provided in various clinical settings.

You will have a weekday schedule (40 hours) rounding on patients in assigned facilities (Skilled Nursing Facilities, Hospitals, Clinics) and providing sub-acute care to patients at post-acute care network SNFs. Some shared weekend call may be required. You will develop patient care plans, collaborating with nursing staff and patient families. You will serve as an authority to facility staff, assisting with organization and quality improvement and teaching nursing staff within these client facilities. You may participate in program development of High Acuity Units in SNF settings as well. This group is ready to hire, so candidates with an Illinois license will receive priority attention.
